This is based on the YouTube version by Flatsound. 
He basically plays slightly altered variations of  all throughout,
I didn't know the proper names so I used a reverse guitar chord finder
but it wasn't exact so I'll tab them out for you.

To reiterate, 
           2 is just a  with an open d string which you hammer on the 2nd fret(* is the hammer on sign). 
           4 is the same as a  except the d string is played on the 3rd fret.
           comes up later and it's like a  except the middle note is on the a string not d
